The following information is included in the Butler County Health Department food establishment inspection reports for November.
Priority items are those that contribute directly to the elimination, prevention or reduction to an acceptable level, hazards associated with foodborne illness or injury, according to inspection documentation. These must receive immediate action within 72 hours or as stated.
Core items relate to general sanitation, operational controls, facilities or structures, equipment design, general maintenance or sanitation standard operating procedures. These items are to be corrected by the next regular inspection or as stated.

Inspections Nov. 1-8
Arby’s
8 core items to be corrected by May 2020:
• Restroom vents are soiled with dust and debris;
• repeat - water from condensation has accumulated in light globe above entrance door inside walk in cooler to be drained/repaired;
• wet soiled wiping cloth found in three-vat sink, is to be in dirty linens/sanitizer solution while not in use;
• repeat - employee personal items found stored on food prep surfaces;
• repeat - soda bags found hooked up to dispenser and directly on floor;
• repeat - debris present on ceiling tiles above soda bag station;
• soda bag station is dripping; and
• lightshields in freezer unit and back storage area broken and/or missing.
Burger King
2 core items to be corrected by May 2020:
• repeat - dust and debris present in and around HVAC vents in main kitchen area and
• repeat - ceiling tiles are out of place over soda bag storage area.
Dexter Bar-B-Que
1 priority item to be corrected, corrected on site:
• Totes of cooked, foil wrapped ribs were found stored in walk in freezer and were not dated or labeled.
6 core items to be corrected by May 2020:
• repeat - mechanical dishwasher doors are soiled with debris and build up;
• repeat - hood vent has grease and debris build up;
• repeat - trays of cooked ribs were found in the walk-in cooler stored openly and unprotected;
repeat - entrance to walk-in freezer is deteriorating;
• HVAC vents to walk-in cooler are soiled with dust and debris; and
• bug fragments present in light shields of main kitchen area.

1 priority item to be corrected, corrected on site:
• cross contamination, raw beef stored over ready to eat products in reach in display freezer.
4 core items, to be corrected by May 2020:
• repeat - there is no approved soap dispenser in the public restroom;
• milk residue in bottom of display cooler in back;
• cobwebs present on ceiling in various areas in retail and stock area; and
• repeat - few ceiling panels gaped open, stained and not properly installed in rear stock area.

1 priority item, corrected on site:
• cross contamination, storing raw chicken over ready to eat food items in display freezer.
4 core items, to be corrected by May 2020:
• restroom vents soiled with dust and debris;
• residue, ice and debris in bottom of display coolers, freezers;
• water stored directly on floor in retail area, to be stored minimum six inches off floor; and
• dumpster lids open at time of inspection
Earl’s Diner
1 priority item, corrected on site:
• cross contamination, raw pork products stored over bread products in freezer unit.
5 core items, to be corrected by May 2020:
• stained ceiling tiles in back area;
• dust and debris accumulated on ceiling, walls around HVAC;
• bug fragments present in light shields in main kitchen area;
• ceiling tiles broken in main kitchen; and
• restroom vents soiled with dust, debris.
